Emergency Bus Plan for AllergiesList Item 4
As the 2026-2027 school year begins we are requesting that you fill out the attached information sheets ONLY IF YOUR CHILD HAS HEALTH CONCERNS AND IS A BUS RIDER. Please return the forms to the school office when completed.

Transportation Change Request
In the event there is a change in afternoon transportation for your child/children, please fill out the form attached and turn it into the main office.

Absentee Form
If your child is absent from school, please fill out the attached form with any doctors excuse attached and turn it into the main office.

Meal Modification Forms
If your child will be eating lunch in the cafeteria and has specific food allergies and/or intolerances, please have your physician fill out and sign this Meal Modification Form. This form needs to be updated annually. A lactose free milk will be provided for any student that has an intolerance.
If your child/children have a request for substitutions that are not medical such as personal, cultural, or religious please use the Non-Medical Meal Modification Form.
In this instance juice or water may be purchased for an additional fee.
